  How to get document on site
Question/Comment:
  I would like to add a survey document to the web page.   I would like parents to fill it out online and me have access to the answers similar to the online registration. Does this work with my site?   I have attached the document.   Elissa Kovala

The next version will have a season evaluation form built in.  In the meantime, there is a charge of about $75 to $100 to create a custom online form.  This type of form will route to you via email.  If you want a form that puts the data into the database, costs would be about $180 to $240.
